Indication:

This drug is used with other HIV medications to help control HIV infection. It helps to decrease the amount of HIV in your body so your immune system can work better. This lowers your chance of getting HIV complications (such as new infections, cancer) and improves your quality of life. Nevimune (Nevirapine) belongs to a class of drugs known as non-nucleoside reverse transcriptase inhibitors (NNRTIs).

Mechanism of action:

Nevimune (Nevirapine) falls in the non-nucleoside reverse transcriptase inhibitor (NNRTI) class of antiretrovirals. Both nucleoside and non-nucleoside RTIs inhibit the same target, the reverse transcriptase enzyme, an essential viral enzyme which transcribes viral RNA into DNA. Unlike nucleoside RTIs, which bind at the enzyme's active site, NNRTIs bind allosterically at a distinct site away from the active site termed the NNRTI pocket.

Nevimune (Nevirapine) is not effective against HIV-2, as the pocket of the HIV-2 reverse transcriptase has a different structure, which confers intrinsic resistance to the NNRTI class.

Resistance to Nevimune (Nevirapine) develops rapidly if viral replication is not completely suppressed. The most common mutations observed after Nevimune (Nevirapine) treatment are Y181C and K103N, which are also observed with other NNRTIs. As all NNRTIs bind within the same pocket, viral strains which are resistant to Nevimune (Nevirapine) are usually also resistant to the other NNRTIs, efavirenz and delavirdine. However, second generation NNRTIs like rilpivirine and etravirine are effective in treatment for HIV strains resistant to Nevimune (Nevirapine) and other first generation drugs in that same class.
